Alexis smuggles Halloween candy into Sam’s hospital room. Despite Sam just having had surgery, Alexis notes her daughter is glowing. Sam flashes her engagement ring. “It’s about time,”
Alexis exclaims. Alexis’ marriages may have been dumpster fires, but she knows marriage will make Sam and Dante’s relationship even better. Sam showed her what a healthy relationship
looks like, and there’s no better demonstration of love than Sam helping Lulu.
In Portia’s office, Trina asks her mother if Heather really left Port Charles. Portia confirms, adding she had nothing to do with it. She then peppers her daughter with questions about school. Trina says everything’s great except for a project partner she hasn’t heard from yet. She assures Portia she won’t let him derail her and leaves.
At home, Willow leaves Nina a message expressing how worried she is that Michael didn’t come home last night. As she hangs up, Drew knocks on the door. “Why would you come here?” she asks. He needs to explain to Michael, but Willow asks him to stay away from both of them. Her marriage can’t take any more of his help. She wanted to tell Michael herself about their kiss, but Drew beat her to it. He explains Ned and Tracy were blackmailing him over it and he had to stop it. All Willow cares about now is her family. She just wants Michael to come home so she can tell him she loves him and will do whatever it takes to earn his forgiveness. She knows Sasha had to have told Cody about the kiss. Drew urges her not to tell Michael about the second one.
Sasha wakes up in bed at the Metro Court as a sleeping Michael rolls over next to her. She flashes back to Michael angrily doing shots at the bar and, later, them making out in the hotel room. When Michael wakes up, they put on robes and rehash their night. They agree they didn’t plan on it and that they’re both to blame. Sasha admits she already knew about Willow and Drew’s kiss. She should have told him. Michael understands she was being loyal to Willow.
As Sasha pleads Willow’s case to Michael, she mentions a second kiss. “There was a second kiss?!” Sasha thought he knew. Michael could wrap his mind around the mistake of one kiss, but a second kiss is definitely a choice. Sasha again defends Willow and feels terrible about having just slept with her husband. Michael says there’s a lot of blame to go around, but he mostly blames Drew.

Dressed as Woody from Toy Story, James wakes up a hungover Cody on the couch. Maxie joins them and sends James off to school. She makes Cody coffee and asks him not to throw up on her rug. Maxie relays that she knows Cody and Sasha are cousins and asks why he went after Drew. He rants about Drew thinking he’s better than everyone else, but he’s really kissing his nephew’s wife and sleeping with her mom. Unfazed since she already knew, Maxie assures Cody that his family will help him through this. Things will get better but he needs to stay away from Drew — or else he’ll have her to deal with. “Is this what having a sister is like?” He quips. “If you’re lucky,” Maxie retorts.
At the surf lodge, Rocco wolfs down his food so he can get to the hospital and see his mom. Dante says she’s still in the ICU, but there’s also something he needs to tell him — he and Sam are engaged. Surprised, Rocco asks, “What about Mom?” Dante recalls how they were sleepwalking after Lulu went into a coma, but Sam unstuck them. It took some getting used to, but Rocco admits most of the time, his new siblings are pretty great. “Sam’s the one who gave us a family again,” Dante says.
Jason steps off the hospital elevator with Danny and Scout, who is dressed as Olympic swimmer Katie Ledecky for Halloween. Jason watches Portia and Elizabeth dash to an emergency. A distraught Alexis comes to the nurses’ hub, and stiffy shakes her head at Jason. Jason sends the kids to Bobbie’s and meets with Alexis. She says she and Sam were just laughing, and then all of a sudden, Sam’s face turned white, and she was confused. Alexis doesn’t know what happened. “I’m scared.”
In Sam’s hospital room, Portia and Liz perform CPR, and Lucas shocks her with paddles. Sam remains unresponsive, and her monitor flatlines. Continuing compressions, Portia says they need to call it. Lucas won’t allow it and keeps working. After more time, Portia tells him it’s been 30 minutes. “She’s gone.” His eyes welling with tears, Lucas somberly says, “Time of death, 5:48 PM.”
